How Much Does Home Care Cost in Northern Virginia in 2026?
Most licensed home care agencies in Northern Virginia charge somewhere in the upper-$30s to mid-$50s per hour for non-medical care in 2026, and the area runs higher than the national average because wages and living costs here are higher. What a family actually pays each month depends far more on hours than on the hourly rate. A few hours twice a week is a very different budget from overnight or 24-hour care.
What drives the hourly rate
The rate covers more than the caregiver's paycheck: licensed agencies pay payroll taxes, workers' compensation, liability insurance, background checks, and the care coordinators who answer the phone at 11 p.m. when a shift falls through. That's why an agency rate is higher than what you'd pay a friend of a friend or someone you found through a neighbor.
The type of care matters too. Companion care (meals, errands, conversation, light housekeeping) usually sits at the lower end. Alzheimer's and dementia care, help with bathing and transfers, or care for someone with complex needs tends to cost more, because it calls for more training and experience. Weekend, holiday, and overnight shifts often carry a higher rate as well.
How hours add up
Families tend to underestimate this part. Here's a rough way to think about it, using a $40 hourly rate purely as an example:
Five hours, three days a week (15 hours) comes to roughly $600 a week, or around $2,400 a month.
Eight hours a day on weekdays (40 hours) comes to roughly $1,600 a week, or around $6,500 a month.
Round-the-clock care, whether live-in or 24-hour shifts, can run well past $10,000 a month.
The jump from part-time to full-time is where most families feel it.
Minimum hours and other fine print
Many agencies require a minimum visit length, often four to six hours, or thirty hours per week, because it's hard to keep good caregivers on one or two-hour shifts with long drives between them. Ask about minimums before you compare rates.
Other things to ask about up front: paid mileage (if the caregiver drives your parent to appointments in their own car, most agencies charge the standard government rate for reimbursement), holiday rates, overtime rates (most agencies will charge time and a half for a caregiver working over 40-hours in a week) how much notice you need to give to cancel a shift, and whether there's a deposit or sign-up fee. A good agency will put all of this in writing before the first visit.
Does insurance or Medicare pay for it?
Usually not. Medicare doesn't cover ongoing non-medical home care, the kind of daily help with meals, bathing, and supervision most families are looking for. Most home care in McLean, Fairfax County, Arlington, Alexandria, and the rest of Northern Virginia is paid privately.
There are a few other sources worth checking. Long-term care insurance often covers in-home care once a policy's conditions are met, so pull out the policy and read the benefit trigger (typically unable to perform at least 2 activities of daily living independently) and elimination period (typically 90 days of service). Veterans and surviving spouses may qualify for VA Aid and Attendance benefits. Some families use a combination: savings, a parent's income, and adult children each contributing a share.
How to keep costs reasonable without cutting corners
Start with the hours that matter most. For many families, that's the morning routine (getting up, bathing, breakfast, medications) or the late afternoon, when confusion and fatigue tend to set in for someone with dementia (known as "sun-downing"). Cover those well first and build from there.
Respite care is another option. A few regular hours a week gives the family caregiver time to work, rest, or just sleep, and it's often what keeps a spouse or adult child from burning out entirely.
Finally, don't choose on hourly rate alone. A caregiver who's well matched to your parent, who stays for months instead of weeks, saves you money and stress that never shows up on a rate sheet.

Frequently asked questions
Is home care cheaper than assisted living in Northern Virginia?
It depends on the number of hours. Part-time home care is usually less expensive than assisted living. Once care reaches full-time or around the clock, home care can cost the same or more, though many families decide being at home is worth it.
Why do agencies cost more than hiring a caregiver privately?
The agency handles workman's compensation, payroll taxes, liability insurance, background checks, etc., and backup coverage when a caregiver is sick. If you hire privately, those risks and responsibilities become yours.
Can we start with a few hours and add more later?
Yes! Most families start small and add hours as their loved one's needs change. Good agencies will check in regularly and tell you honestly when it thinks more or less care makes sense. Why does my dad seem so agitated in the afternoon? Why does my mom always want to go home at 4:00pm when she is at home already?
Dementia patients often "sun-down" which is a clinical phenomenon characterized by a distinct pattern of increased confusion, anxiety, agitation, restlessness, and behavioral changes that occurs during the late afternoon, evening, or night.
Is there anything we can do to help with sundowning?
Yes. Sometimes what seems like "sundowning" could actually be the person trying to communicate a need like needing to use the restroom or hunger or pain or processing something from earlier in the day that bothered them. Uneven indoor lighting can cast bothersome shadows or cause confusion. If they are agitated, try distracting them with a favorite memory from the past or softly playing some of their favorite music.
